Admissions

We at Valley Home Assisted Living & Memory Care take a team approach to the care of our residents.

We encourage family involvement in the care by having them take their loved ones to appointments, accompany them to ER or Urgent Care appointments, and visit any time.

We do admissions per the families needs as not all families are able to leave work to do the admission paperwork.  We do require that all potential admissions have a History and Physical no more than 30 days prior to admission, a list of current medications, an order from the primary physician that assisted living is appropriate for the resident.  For the Memory Care Unit, the potential resident must have a diagnosis of dementia upon admission.

If the admission is to be on a week-end the medication has to be brought into the nurse so that it can be checked and matches the doctor’s orders.

We have an open door policy at Valley Home and encourage all concerns and questions be directed to our Administrator or to our Nursing Care Coordinator Supervisor.
